Abstract

Electronic survey instrument disseminated through SurveyMonkey used to collect data from Illinois collecting institutions as part of the its Connecting to Collections Statewide Planning Grant. The questionnaire was based on the Heritage Health Index survey developed by Heritage Preservation and edited by the Minnesota Historical Society. It was further adapted for use in Illinois. The survey was conducted from August 1, 2010-October 31, 2010.
