- Incomplete tumor resection remains a pervasive issue in modern medicine. Near infrared fluorescence image guided surgery is an effective supplement to surgeons’ skills that has helped to reduce positive margin rates and post-operation morbidity, but modern instrumentation suffers from coregistration errors, inability to image in real time, optical complexity, and large system footprint. To address these issues, we present a 720x1280 hexachromatic near infrared imager with independent readout of color and near infrared channels, 50% quantum efficiency at 700 nm, and a limit of detection of 298 pM of ICG dissolved in 90% FBS. Having three channels in the near infrared allows spectral discrimination among different fluorescent probes which contemporary systems cannot achieve. The use of multiple near infrared probes enhances the capacity of our system to detect and correctly identify tumorous tissue with greater confidence than is possible with only a single marker. The feasibility of multiprobe imaging was shown in a clinical study where our imager was capable of identifying 93% of sentinel lymph nodes using methylene blue and indocyanine green jointly. The ability to image multiple fluorophores and avoid the pitfalls of current technologies used in the operating room promises to improve cancer detection capabilities along with patient outcomes long-term.
